The Continuum of Care

The Continuum of Care model of service delivery is based on a US Department of Housing and Urban Development (HUD) concept.  At Community Family Life Services, we put the Continuum of Care into practice as a holistic approach to providing services, meaning that we attempt to  meet each individual or family where they find themselves in life when they seek our assistance. 

Our services work alone and together to offer as much assistance as we can to the families and individuals who seek our help.  Each individual and their family may receive any or all of our services--each aimed at addressing an underlying cause or factor of poverty and homelessness. 

How our Continuum of Care helps the families, individuals, and children we serve...

...a family who is homeless, where the mother has been out of work and the children need help in school, might join our housing program.  The mother would have access to job training through our programs, and the children would enroll in our after school tutoring programs.  The mother would receive counseling through our Health and Wellness program as part of both our housing and job training support services.

...a formerly incarcerated young, single man who used to be involved with drugs, but who is now clean, might move into our Milestone Place Single Room Occupancy Facility.  There he would work closely with our counselors to make sure that he stays drug-free and might join our WAVE classes to obtain his GED.

...a family where the head of household has recently lost his or her job might look to us for a bag of groceries and access to our Employment Resource Center and employment counselors to begin the process of searching for a new job.
